Medication Therapy Management

Obesity has become an epidemic which leads to diseases like type 2 diabetes, heart disease, high blood pressure, stroke, high cholesterol and certain cancers. Physicians place patients on medications to “control” these diseases — often as an unfortunate quick-fix that introduces a myriad of side effects. The “latest and greatest” medications often come with higher out-of-pocket costs that lead you to pay thousands of dollars annually while watching your conditions worsen.

Some physicians receive kickbacks from these drug manufacturers, while others simply fail to keep up with the latest research and guidelines. Our pharmacists are medication experts engaged in ongoing education that keeps them abreast of the latest developments.

But the unfortunate truth is, most medications are not designed to cure your condition. Because the pharmaceutical industry monetizes off of their patients, it’s not actually in their interest to provide a cure. Medication Therapy Management (MTM) helps direct you toward the solutions that extend your life span, limit your reliance on prescription drugs, and improve the quality of your life. MTM allows our pharmacists to collaborate with your physician and ensure that your medications are:

  • Appropriate and Effective for your medical condition.

  • Administered at the correct dose and at the correct time of day.

  • Safe in light of other conditions.

  • Safe in light of other medications you may be taking.

  • Safe in light of any dietary changes.

  • Safe in light of any lifestyle modifications you may be making, such as beginning an exercise program.

  • Cost-efficient for your condition.

What are the steps to MTM?

Medication Therapy Review a systematic process of collecting your specific health information, assessing medication therapies to identify medication-related problems, developing a prioritized list of these problems, and creating a plan to resolve them.

Intervention and/or Referral once we identify a medication-related problem, we inform you of the issue and, if necessary and with your consent, reach out to your physician to resolve the problem. If a highly specialized medical condition or therapy extends beyond the scope of our program, we refer you to other healthcare professionals qualified to address it.

Implementation and Documentation

  • Medication-Related Action Plan (MAP) details a list of your medication-related problems and actionable steps to address them. This is used to track your progress as you go through the program.

  • Personal Medication Record (PMR) — a comprehensive record of your medications (prescriptions medications, nonprescription medications, herbal products, and dietary supplements) which you’re encouraged to maintain and update. Share this document at all healthcare visits to keep practitioners aware of your current medication regimen.

Follow Up we recognize the difficulty of making lasting lifestyle changes, and we provide the support to help you do so. We follow up at specific intervals to track your progress and address any issues or barriers you may be experiencing. We will also act as the middleman between you and your physician to ensure all members of your care team stay abreast of your progress.

When you make the commitment to embark upon a program that addresses lifestyle factors such as exercise, diet, sleep, and stress management, you will see measurable improvements to your chronic conditions. MTM will optimize your medication regimen and then reduce or eliminate that medication as your health progresses.
